The book which used this cover is named "Wege des Meisters" (engl. "Ways of the [Dungeon] Master").
As Traviadan explained, the hands pulling the strings are those of the dungeon master.

@wahay: Isn't that obvious? It's a cover for a RPG-Supplement. The gamemaster plays the ogre and the orcs like puppets on a string. The four figures in the foreground are the heroes played by the players. The player of the female mage hasn't yet "told" his heroine how to react to the threat ahead, so she is looking behind her asking: What shall I do?

The question I have is: Why aren't there strings from the heroes going towards the viewer? :-)

I agree with wanwan—that background has barely any colour in it but still looks brilliant!

Why is the woman looking behind her? I don't think she's being ambushed, because she's not really looking surprised.

I like the orcs - but I think the ogre (?) is a bit too small and not ugly enough.

i am in awe of this, i find it pretty hard to work in a fairly low saturation palette - good job sir!
